Bloomberg, in this report, says that “a few hours after the February 28/March 9 missile attack that targeted a school in Iran, some Pentagon staff knew that the United States was responsible for the attack. This was stated by officials who were involved in an internal investigation into the incident.”

Based on this report, “the investigation has revealed what these officials now describe as a ‘chain of failures that could have been prevented.’”

On the day the war began, two Tomahawk missiles targeted the building of the elementary school Shajara-e Tayyiba and its compound, killing more than 150 people, including at least 123 children.

The attacks destroyed the two-story school built on land that was part of a military complex in the southern city of Minab.
